1. Program Description

FloorTek is a sales operations management platform used by OX Dealer Services, LLC and its authorized dealer network (including FloorTek Concrete Coatings and OX Floors) to manage flooring installation appointments, pricing, and contracts. This SMS messaging program sends transactional notifications to authorized sales representatives and managers regarding:

• New offer submissions from sales representatives
• Offer approvals and rejections by managers
• Counter-offer notifications
• Appointment scheduling updates and changes
• Contract signing notifications
• System alerts and operational updates

2. How You Opt In

End users consent to receive SMS messages through an explicit opt-in process within the FloorTek mobile application. During account setup, each authorized sales representative or manager is presented with an SMS consent screen that displays the following consent language:

"By providing your mobile number, you agree to receive transactional SMS notifications from FloorTek regarding offer submissions, approvals, counter-offers, and appointment updates. Message frequency varies based on activity. Message and data rates may apply. Reply STOP to cancel or HELP for assistance."

The user must check a consent checkbox and tap "Agree & Continue" to activate SMS notifications. Consent is recorded in our system with a timestamp and the user's account ID. Users who do not complete this opt-in step do not receive any SMS messages.

3. Message Frequency

Message frequency varies based on sales activity. Authorized users typically receive 1–10 messages per week, though volume may increase during active business periods. Messages are sent only when triggered by specific operational events (e.g., a new offer is submitted, an offer is approved, or an appointment is updated).

4. Message and Data Rates

Standard message and data rates may apply depending on your mobile carrier and plan. FloorTek and OX Dealer Services, LLC are not responsible for any charges incurred from your mobile carrier for sending or receiving SMS messages.

5. How to Opt Out

You can revoke your consent and stop receiving SMS messages at any time by using any of the following methods:

• Reply STOP: Reply STOP to any SMS message you receive from FloorTek. You will receive a one-time confirmation message and will no longer receive SMS notifications.
• In-App Settings: Disable SMS notifications in your FloorTek app settings under your account profile.
• Contact Support: Email us at [email protected] to request removal from SMS notifications.

7. Who Receives Messages

Messages are not sent to the general public, external consumers, or any individuals who have not completed the in-app opt-in process. Only users with active accounts and valid mobile numbers who have explicitly consented receive SMS notifications.

8. Data Protection

We take the protection of your information seriously. Your phone number and SMS opt-in consent data will not be shared with any third parties for marketing or promotional purposes. SMS consent data is stored securely and used solely for the purpose of delivering transactional notifications through the FloorTek platform.

9. Supported Carriers

SMS messages are supported on all major U.S. mobile carriers, including but not limited to AT&T, Verizon, T-Mobile, Sprint, U.S. Cellular, and their MVNOs. Carrier support may vary and FloorTek is not responsible for delayed or undelivered messages due to carrier issues.
